Our custom scorecard is an option on our Elevate Platform and is perfect for clients with an incentive-based wellness program.  We work with you to determine target measures specific to your employees’ risk factors.  Our scorecard tracks Biometrics, Health Risk Assessment Completion, Tobacco Status, or Tobacco Affidavit completion.

• Individuals view their activity status from their personal dashboard in our Elevate Platform and can be downloaded as a PDF

• Scorecards can be branded with client logo or wellness program brand

• Reporting based on: pass/fail; completion status; incentives reached; or results-specific data
