We offer a comprehensive health risk assessment (HRA), created by the experienced physicians at Be Well Solutions. All questions are based on published research linked to health outcomes and incorporate national health standards and guidelines.

About Our HRA

  • Takes less than 10 minutes to complete – sections can be turned off/on, depending on the length that best suits your company
  • Available in online or paper versions
  • Written at an 8th grade reading level
  • Available in English and Spanish

HEALTH RISK REPORT

Upon completion of the HRA and biometric screening, each participant receives a 6-page Health Risk Report that helps identify potential health factors in each of the five key risk categories.  Risk levels in each category are identified for participants as LOW, MODERATE, or HIGH.
