Your biometric screening results (such as blood pressure, cholesterol, glucose, height, weight, and BMI) and/or health assessment results are collected only to give you personalized health feedback and to improve your wellness program.

Your biometric screening results are confidential and will not be shared with your employer or used for any employment-related decisions. Your employer may receive only group-level information (such as averages or participation counts) to help with wellness program planning. Your personal health information will not be identifiable in these reports. For billing purposes, your information may be shared with your health insurance provider.

Your information is stored in a secure, encrypted system in compliance with HIPAA security requirements and is only accessed by authorized staff who have completed HIPAA privacy and security training. You can request a copy of your results, ask for corrections, or withdraw consent at any time (within legal limits). We keep your data only as long as needed to fulfill program purposes or as required by law, then securely delete or de-identify it.

If you click on any websites that are linked to or from our portal or website, we cannot ensure the protection of any personal information that you provide to a third-party site, as these third- party sites are owned and operated independently. Third-party sites may have their own privacy and data collection practices that are beyond our control. Any personal information that you provide to these sites will be regulated by their privacy policy, if any. We can assume no responsibility or liability whatsoever for the practices, actions, or policies of third- party sites, and cannot be responsible for the content or privacy practices of such sites.
