HealthWorks is an independent company specializing in health improvement programs. HealthWorks conducts health screenings and Health Risk Assessments (HRAs) to help you manage and improve your health. These programs are administered by HealthWorks and are completely confidential. HealthWorks maintains strict confidentiality of all participant and client information and affairs. We take many steps along the way to ensure all information obtained through our health screen process remains completely confidential. We handle all confidential information in accordance with the current HIPAA guidelines.

We have in place a system of physical and electronic procedural safeguards to protect the confidentiality of your information. We are continuously evaluating and implementing new procedures to ensure the utmost integrity of our processes.

The following are steps we have implemented to ensure your information remains safe and private throughout the entire process:

• Appointment scheduling is completed through a secure password protected website. As you schedule your appointment the screen will simply list what appointments are booked or available. Individual names or personal information is never made available to the public.
• Nursing staff and technicians are trained to be discrete when documenting clinical information, such as height/weight and blood pressure readings.
• Health Risk Assessment data is stored on a password protected computer.
• No personal health information is transmitted via the internet.
• Personal Wellness Profile reports are mailed directly to your home.
• Personal health information is never shared without your written consent.
• HealthWorks compiles all HRA and screening data into a group health risk report, which does not allow identification of any individual information. Only group summaries are provided to employers for the purpose of assisting them in planning future health and wellness programs.
• HealthWorks does not retain copies of documentation containing personal information. All copies are mailed to the participant. Once the job is completed, the hard copy questionnaires are shredded.
